Hazardous Waste Approvals
Requirements – PA Module 1 Form, PA Form 25R, Generator’s Knowledge Letter, Module 1 Analytical, Sample for MAX’s Treatability Study,
PA Module 1 – The PA Mod 1 form is required for all new approvals. The form is to be completed, signed and notarized.
PA Form 25R – This form is the Source Reduction Strategy. The generator should complete as thoroughly as possible and be sure to sign the document in the upper right corner of page 1. Hint: for one-time clean up projects, the source reduction is not always applicable, however any methods used to reduce the amount of off-site disposal should be included.
Generator’s Knowledge Letter – This letter is used to identify what the generator says is NOT in the waste. The appropriate identified chemical constituents should be denoted with a mark next to the name of the chemical (X or check mark). The generator can copy the enclosed letter on to the generator’s letterhead . The letter needs to be signed and dated. For ongoing projects or waste streams, an original will be required for the annual update of the waste stream.
Module 1 Analytical – All waste streams are to have analyses run on the samples. The requirements for soils and for regularly generated waste materials are attached. Note: MAX can have the analytical services performed for the customer.
Treatability Study – Each new waste stream is required to have a treatability study performed by MAX as part of the approval process. A 1-gallon sample should be sent to MAX’s Yukon Facility (address above). Samples should be marked:
Attention: Laboratory Manager. Included should be a chain of custody form. This form must contain the name, address, and EPA ID number for the site the waste is being shipped from. If the EPA number has not yet been assigned.
Non-Hazardous Waste Approvals
Requirements – PA Form U, PA Form 25R, Generator’s Knowledge Letter, Module 1 Analytical.
PA Form U – The enclosed PA Form U is required for all new approvals at Yukon's Subtitle D landfill.
